ATHLETIC BOOSTERS SCHOLARSHIP - $750 AWARD

Please complete the following application by typing the answers on a separate sheet of paper and

Qualifications to be considered for the award are:

1. 2.5 GPA or higher
2. Must have participated in two varsity sports over your high school career & received two varsity letters.

The check will be made directly to the college the applicant has been accepted to.

All information must be typed on a separate sheet of paper.

Please indicate your GPA.

Answer the following questions:
1. Please provide a summary of your involvement in athletic activities and list related awards that you have received.
2. Please share with us your plans for the next four years after graduation.
3. What are your long-range goals or where do you see yourself ten years from now?
4. Please explain the importance of high school athletics in relationship to your involvement.
5. Write a brief essay on the importance of being an athletic role model.
